
I was so excited to hear that Kelly Howard opened her own women’s health clinic. Today was my first visit and I was greeted by the most friendly staff and the facility is so comfortable. They have so many services and are adding more each day. If you are looking for a new women’s health provider I would definitely recommend Shoals Women’s Health and Wellness.

When can I request a prescription refill?
Current patients can request refills according to specific needs. Please call our office at (256) 448-9500 if you have any questions regarding refills.
Please allow 24 hours for refills to be called into your pharmacy once you’ve spoken with us and a refill has been approved.
Because of insurance requirements, please allow up to 48 hours for a Prior Authorization (PA) to be completed and faxed to your pharmacy.
Please bring all medications with you to your appointment, or at the very least, a detailed list of your medications.
You can also fax a list of updated medications to 256-448-9999 or update your intake form at your next visit.

How often should I update my information?
Patients are required to have their patient demographics updated on an annual basis. This includes patient information, guardians who can bring a minor in for their appointments, emergency contacts, pharmacy information, insurance information, and acknowledgment of patient policies/procedures (see Patient Forms section below). Signed HIPAA form.
Required at first visit: valid driver's license and insurance card. Patients with name or custody changes must present documentation. If there is a name change, ensure it is updated with your insurance carrier.
